Epidural corticosteroid injection for management of chronic lower back pain

Epidural steroid injections are commonly used to treat back pain caused by a herniated disc (slipped disc), lumbar radiculopathy, spinal stenosis or sciatica. Corticosteroids are strong anti-inflammatory medications and, when placed into the epidural space, can significantly reduce inflammation around an irritated nerve that is causing back and leg pain and discomfort.

Patients with several common conditions – including lumbar disc herniation, degenerative disc disease, lumbar radiculopathy, sciatica, lumbar spinal stenosis, may benefit from an epidural injection.

Epidural steroid injections work by delivering a potent anti-inflammatory to the site of nerve impingement in the spine. Injected medications may include steroids, local anesthetics, and saline, and they can vary in volume and concentration on an individual basis.

Research highlighted, Epidural corticosteroid injections can be recommended as additional therapy only in the acute phase of the conservative management of lumbosciatic pain.

According to the literature analyzed in this narrative review, there is no consensus on the use of ESIs for patients with chronic lumbar pain. ESIs seem to be effective in relieving symptoms in the short term and delaying surgery, while evidence of any long-terms benefits is still lacking. More studies are needed to better understand which patients could benefit more from epidural steroid injections.
